Counselling Request Form

Counselling Support

To book a counselling appointment, please complete and submit this form.

Once your form is received, you will be offered a time for a counselling session. This first session can last up to one and a half hours with any subsequent sessions lasting up to 50 minutes. This first session is for you to talk about why you have contacted us and to exchange information.

The counsellor will ask some questions about your previous medical and mental health history and about your current situation. It is also an opportunity for you to ask questions about the counselling process, how the counsellor works and what to expect from it.  You then decide together whether further sessions would be helpful and appropriate. If so these are booked into the calendar and an invitation emailed to you.

All sessions are strictly confidential unless there is a risk to you or someone else, in which case the counsellor will explain that certain information will have to be shared with the appropriate authority for your own safety.

IMPORTANT NOTE: The CFS counselling service is not an emergency service. If you, or someone you are with, is in crisis please dial the emergency NHS no 999 or go to your nearest Accident and Emergency.

You can also phone Samaritans on 116123
